Agreement for paid use of PlusMed service to receive advisory medical services — PUBLIC OFFER
This document is a public offer with appropriate legal force, regulating the rules for using the PlusMed service, represented by UNITX LLC, addressed to an unlimited number of persons, regarding the intention to conclude an Agreement for the provision of advisory medical services through the PlusMed service under the conditions described below.
The parties to the Agreement are UNITX LLC and an individual seeking consultation/direction/conclusion for health problem resolution, either in their own interests or in the interests of a third party (hereinafter 'Patient').
User familiarization with the offer is carried out by posting the current version on the Website at: www.plusmed.uz
Full and unconditional acceptance of the terms of this Offer is considered to be the User's actions: registration on the Website, marking consent to the terms of this offer including the company's privacy policy. In case of consultation with a specialist — prepayment for PlusMed Services.
The Agreement is considered concluded if the parties have reached agreement on all essential terms of the contract. The Agreement is concluded with each user in case of applying for PlusMed Services.
The provision of services to the User is carried out subject to the execution of informed voluntary consent for advisory consultations in the form of an electronic document.
The User can receive anonymous paid advisory primary consultation using telemedicine technologies. The procedure for anonymous receipt of Services is available in the Personal Account.
The User consents to receiving informational and advertising messages via SMS and Email.
The User agrees that the offer may be changed unilaterally by PlusMed with notification via SMS or website www.plusmed.uz no later than 30 days.
For the purposes of this Agreement, the following concepts are used:
The Doctor undertakes to provide paid primary services for oral or written consultations of an advisory nature. The User pays for services according to the agreement.
Doctors provide services remotely using PlusMed. The User consents to the debiting of funds from the card for payment of services. The consultation is advisory in nature, and the user bears responsibility for independent treatment actions.
To receive services, the user registers on PlusMed. Information about doctors and the cost of services is available on the website www.plusmed.uz. The service is considered rendered if within 7 days the user does not file a claim. In case of refusal or absence online — penalties of 2-20% apply.
Services are provided online using PCs, mobile devices, with camera and microphone. The User is responsible for the compliance of equipment with PlusMed requirements.
The cost of services is indicated for each doctor in their profile. Payment is made cashless to the account of UNITX LLC. The service is considered paid after funds are credited.
PlusMed undertakes to keep patient information confidential; transfer is possible only with the patient's consent.
Doctors are responsible for the quality of services. PlusMed is only responsible for the quality of online communication between doctor and patient.
No party is liable for non-performance of obligations due to force majeure circumstances. Performance deadlines are extended for the period of force majeure.
The Agreement is public, indefinite, may be terminated with 1 day notice. All additional agreements are an integral part. Disputes are resolved first through negotiations, then through court.
